We are pleased to announce that we received our accreditation from the Board of Certification/Accreditation (BOC) and also met the “State of Florida Surveyor Guidelines” to be in accordance with ACHA.

BOC standards meet the DMEPOS accreditation requirements under section 1834 (a) (20) of the Social Security Act (SSA) and the federal regulations at 42 CFR 424.58. BOC based its decisions on a review of our application and site survey conducted on November 27, 2017 where the “Florida Surveyor Guidelines” were utilized.

BOC approved Moonlight Orthotics LLC, whose NPI number is 1588297824 for an effective date of
March 19, 2020, through March 31, 2023, for the following DMEPOS product categories:

DM05,DM06,DM08,OR03,PD06,S01,PD09
